Patient Forms
Click on the button below to print your New Patient Forms and complete them before your first visit. If you have seen Dr. Mehta in the past, but have not yet had a telemedicine visit, please fill out the Telemedicine Consent form before your first telemedicine visit.
Accepted Insurances
We accept most major insurances at Uptown Allergy & Asthma. You can review our complete list of accepted insurances here. If you have questions about your coverage, please contact us.
What to Expect
On your first visit, Dr. Mehta will review your history, focusing on whatever your main concerns are. This is followed by a physical exam and diagnostic testing. The first visit typically lasts about 45 minutes to 1 hour and the patient will leave with a detailed treatment plan that will improve his or her symptoms.
Allergy and asthma testing
Generally, skin testing for allergies is performed on the first visit but sometimes blood tests are ordered depending on the nature of the problem. Skin testing consists of scratching the skin and is not painful (but can itch). Pulmonary function testing is also performed if a patient has respiratory issues.
Medications that interfere with skin testing
Please review our list of medications that interfere with skin testing well in advance of your first visit. Certain medications must be stopped for 5+ days prior to skin testing. If you have any questions about your medications, please contact us for guidance.
What to bring for your first visit
(1) Completed new patient forms, (2) Your current insurance card, (3) Physician referral forms (if required by your insurance carrier), and (4) any past records, including blood tests, past allergy testing, recent imaging or other relevant documents.
Please use our parking lot!
We have parking available in our reserved lot behind the office. For your convenience, we recommend that you use this parking whenever you visit us at Uptown Allergy & Asthma.
Don’t be late!
We will not be able to see you if you are more than 15 minutes late for your appointment. Please be on time, or we will have to ask you to reschedule.
